Important Things to Know for 2009-2010
IMPORTANT: ALL HEALTHCARE REIMBURSEMENT PARTICIPANTS WILL RECEIVE A NEW FLEX CARD FOR THE PLAN YEAR BEGINNING 9/1/2009. “BENNY” FLEX CARDS ARE NO LONGER ACTIVE AS OF THE LAST DAY OF THE PRIOR PLAN YEAR, 08/31/2009. CLICK ON BENEFITS AND FORMS/REIMBURSEMENT ACCOUNTS FOR MORE INFORMATION.
The East TX Coop Cafeteria plan year is September 1, 2009 through August 31, 2010. Supplemental benefit changes become effective October 1, 2009. Online enrollment for the 2010 plan year will begin May 1, 2009 and end on May 29, 2009.
You must complete your online enrollment within this enrollment period. Flexible Spending Accounts (FSA): The plan year for FSA Medical and Dependent Care accounts is September 1, 2009 through August 31, 2010. Please plan your contributions carefully, as these accounts are Use it or Lose it!
Flex Cards:If you currently participate in the Medical Reimbursement accounts, Benny Cards are valid for 3 years, so keep your Benny card! The card will be re-loaded with your new FSA election amount in the new plan year; you will not receive new cards.
Dental: New enrollies will have a 12 month major services waiting period.
Disability: Pre-existing condition exclusions will apply for new coverage or increases in coverage. For details, see the plan brochure by clicking on benefit highlights/disability.
Voluntary Term life: Employees enrolling in the plan as late entrants (adding plan or increasing benefit amounts) must complete evidence of insurability to be considered for coverage under this plan (a physical may be required).
Plan offerings to East TX Coop employees include: Oraquest Dental Plan, UHC/Spectera Vision, Unum Educator Select Disability,American Public Life Cancer plan, Unum Group Term Life, Tx Life Individual Permanant Life and Flexible Spending Accounts 9not all district apply).

Important IRS Notice: New mandated federal regulations require all dependent socials for any elections made for medical, dental, and/or vision. All employees are required to login and update their dependent information prior to the plan effective date 9/1/2009. To update your dependent information in the system click dependent information and select the pencil to edit. To log in go to www.ETXEBC.com and click on

corner.  Your username is the First 6 letters of your last name,
Your First initial followed by the last 4 digits of your
Social Security Number. Your password is your 9
